Respiratory: COPD Virtual Wards in the Community

Region: Ireland

myPatientSpace has implemented two major COPD community virtual wards in Ireland.

  • Letterkenny Care VW
  • Galway COPD VW

Patients as provided with remote monitoring kits, with a variety of devices depending on the clinical preferences (Continuous RR, Sp02, BP, Temp). Within the first 6 weeks of go live they saved over 70 bed days resulting in estimate savings of over 70,000 euro. The projects have won top European awards and been invited to speak internationally on their program at academic conferences.

Program to remotely manage patients on an island off the coast of Ireland. Providing direct access to GP services and support patients with hypertension.

myPatientSpace has been used in many very successful oncology remote pathways.

 

  • A pathway for menopause after cancer was presented at ASCO/ESMO and published in the cancers journal. Remote management and research study for cancer survivors.
  • Geronte: myPatientSpace is the selected platform for a large H2020 Cancer project for geriatric oncology enabling remote management
  • Live in a number of leading OECI cancer centers (including RCSI) for remote patient management

Chronic Kidney Disease

Region: Ireland

myPatientSpace is used to remotely manage over 2700 CKD patients at St. James hospital that top academic teaching hospital. It has not been rolled out to other sites in the country following this success and integration to the national renal system has begun. This is enabling new models of care across the Renal journey.

Chronic Pancreatitis: SmartCP

Region: Ireland

Tallaght (Trinity College Academic hospital) has won the top Innovation in hospital management award for Europe with this program. This remote management of Chronic Pancreatitis patients across the country. This is now innovating new models of care including PIR models to reduce outpatient demands.

ResMed PEI delivers a managed care service for patients on ResMed CPAP devices. All patients in Ireland use this service and the mypatientspace platform.
