We are proud to announce that the Smart CP platform, developed by myPatientSpace in partnership with Tallaght University Hospital, has been awarded the Best Digital Health Innovation Award at the HSE Spark Summit 2026 Bright Spark National Innovation Awards.
The award recognises Smart CP’s innovative approach to supporting people living with chronic pancreatitis through a Digitally Enabled Patient-Initiated Review (PIR) model, helping patients access care when they need it while improving efficiency across healthcare services.
Transforming Chronic Pancreatitis Care
Managing chronic disease continues to place increasing pressure on healthcare systems, with growing outpatient waiting lists, routine follow-up appointments and limited capacity to respond quickly to patients requiring urgent care.
At Tallaght University Hospital, many patients living with chronic pancreatitis experienced significant delays before receiving specialist review.
Smart CP was developed to address this challenge.
Through the myPatientSpace app, patients can:
- Track symptoms and complete patient-reported outcome measures.
- Communicate directly with their clinical team.
- Trigger a clinical review when needed.
- Self-schedule appointments where appropriate.
Rather than relying on routine clinician-led follow-ups, the platform enables a proactive, patient-centred model that improves access to care while helping clinicians prioritise patients based on clinical need.
Delivering Measurable Impact
The results achieved through Smart CP demonstrate the value of digitally enabled patient care.
Since its implementation, patients have:
- Submitted more than 14,000 patient-reported outcome measures.
- Sent over 1,200 direct messages and other interactions to their clinical team.
The programme is projected to:
- Avoid more than 1,050 outpatient appointments over three years.
- Deliver approximately €210,000 in healthcare cost savings.
- Increase clinic capacity.
- Reduce unnecessary travel and associated emissions.
- Improve the overall patient experience.
These outcomes highlight how digital health can improve both clinical efficiency and patient engagement while supporting more sustainable healthcare delivery.
